Read moreThe Health Services Safety Investigations Body (HSSIB) is an independent arms-length body of the Department of Health and Social Care, governed by our Board.
Our Board is made up of people from diverse backgrounds, including healthcare, law, finance and IT. They bring with them different experience and perspectives that support HSSIB’s mission. They are actively involved in our strategic development and performance management.
The Board is led by Ted Baker, Chair. He is joined by five non-executive directors and four executive directors.

Our Board is committed to accessibility, openness and transparency. We hold our meetings at host organisations across England and encourage our healthcare sector colleagues and the public to attend and observe.
If you wish to attend a Board meeting, either virtually (using Microsoft Teams) or in-person, please contact executiveoffice@hssib.org.uk at least five working days before the meeting. Please let us know if you have any specific requirements.
If members of the public arrive after the meeting has already started, unfortunately it may not be possible for them to join the meeting. Please be aware that questions cannot be asked during the Board meeting. Once the meeting has ended, members of the public can ask our governance team any questions related to the business considered at the meeting or email questions to executiveoffice@hssib.org.uk.
The next HSSIB Board meeting takes place on Thursday 31 July 2025, 9.30am-12.30pm, at the Royal College of Physicians, Liverpool.
