The Knowledge Exchange aims to be a central source of information to help any person who is involved in patient safety investigations. This could be a patient safety specialist, a patient, a family member, a member of staff or a commissioner.
The resources aim to be engaging and meaningful. They give real-world insight into completing patient safety investigations in a professional, informed and compassionate way.
Resources will be expanded and developed over the coming years.

In 2026, we launched Building investigation excellence: our strategy for strengthening the capability of healthcare investigations. The Knowledge Exchange is part of our offer to support the building of investigation capability across healthcare, including the NHS in England.
