How we learn about patient safety issues

We learn about patient safety concerns in a number of different ways. Some examples include:

  • information from patients and members of the public
  • information from healthcare staff and organisations
  • patient safety data
  • healthcare publications
  • Coroner's reports.

We do not investigate individual patient safety events or concerns. Instead, we bring together the range of information and feedback we receive to help us understand risks and potential areas for improvement across the healthcare system. This information allows us to identify the patient safety issues that could benefit from a Health Services Safety Investigations Body (HSSIB) investigation.
