Request

I am making a request under the Freedom of Information Act 2000 in relation to the former Healthcare Safety Investigation Branch (HSIB) investigation: Timely detection and treatment of cauda equina syndrome.

I would be grateful if you could provide the following information:

  1. The names and job titles of the HSIB employees who formed the investigation team.
  2. The name and job title of the lead investigator.
  3. The names, professional roles, and employing organisations of the three subject matter advisers referred to in the published report (the consultant spinal surgeon; the consultant in emergency medicine; the advanced physiotherapy practitioner for spinal services and the emergency department).
  4. The investigation reference number.
  5. The dates on which the investigation formally commenced and concluded.

If any information is withheld under the Freedom of Information Act, please identify the exemption relied upon and provide any non-exempt information that can be disclosed, including job titles, professional roles, and employing organisations where names cannot be released.

Decision

HSSIB is only able to undertake FOI requests for information in relation to HSSIB which was established in October 2023. The investigation that you are enquiring about was undertaken by HSIB in 2020/2021.

Details about the HSIB legacy can be found on our website.

Any FOIs in relation to HSIB need to be redirected to the FOI team at NHS England (as HSIB was part of NHS England when it was in existence).

Full details of their FOI process can be found on their website. For convenience though, their email address is: england.contactus@nhs.net.

However, in the interests of transparency, we would like to note that a copy of the final report is available in the legacy section of our website. The investigation commenced on 11 March 2020 and concluded on 19 August 2021, when the report was published. The investigation reference number is I2019/015.
