HSIB makes the following regional safety recommendation

HSIB recommends the Acute Trust work with the Ambulance Trust to develop and implement a standardised approach to verifying and confirming a patient’s identification during the handover process.

Response:

The Acute Trust responded to this safety recommendation and described their following actions:

  • Exploration of system functionality for automatic transfer of an incoming patient's NHS number from ambulance to emergency department systems.
  • Ensuring clinical staff are aware of the potential consequences of poor patient registration and issues at handover.
  • A standard approach to handover is in place.
