HSIB recommends that the Acute Trust develops and implements a standardised approach to patient identification in the emergency department.

Response:

The Acute Trust responded to this safety recommendation and described their following actions:

  • Reinforcing of correct processes around patient identification for emergency department receptionists.
  • Review of Trust systems to ensure software and identity bands support the checking process.
  • Working with the Ambulance Trust to support handover of correct patient information.
