I would be grateful if you would provide a list of services and of anonymised incidents, involving NHS or NHS contracted services or staff, that you have investigated or are now investigating with regard to inpatient or outpatient treatment for anorexia nervosa provided by or funded through NHS related bodies together with any findings from those investigations.
