After Action Review summary report template

The After Action Review (AAR) summary report template has been co-designed by NHS England, the Health Services Safety Investigations Body and staff leading AARs in a range of healthcare organisations.

It was developed to standardise the reporting of AARs. It is not intended to be an AAR facilitation guide. The structure is purposefully simple so that AARs can focus on reflective conversation and do not become a bureaucratic documentation exercise.

This structure will continue to be evaluated and developed in response to feedback and learning about its use in practice.

Learning Response Review and Improvement Tool

The 'Learning Response Review and Improvement Tool' is intended to be used by:

  • Those writing learning response reports following a patient safety incident or complaint, to inform the development of the written report.
  • Peer reviewers of written reports to provide constructive feedback on the quality of reports and to learn from the approach of others.

The tool was developed based on research undertaken by NHS Education for Scotland. It has been refined by HSSIB (previously HSIB) and NHS England. The content of the tool was validated with users in a study led by the University of Reading.

This webinar took place on 23 January 2025.

This highly practical webinar was for those who undertake After Actions Reviews (AARs) or were planning to do so. It was of particular interest to those using AAR as a learning response under the Patient Safety Incident Response Framework (PSIRF).
